ABSTRACT

Abstract The aim of this study is to provide a real picture of the disease burden of Prameha in society. The study was performed in Government Ayurved College and Hospital, Nagpur, Maharashtra during Oct 2015-Mar 2016. Total 60 patients of newly diagnosed type 2 diabetes mellitus attending the Kayachikitsa Opd of GAC Nagpur were included for the study. The subjects details were recorded in case report form. The CRF included many variables such as sociodemographic factors, presenting symptoms, risk factors such as hypertension, obesity and glycaemic status, family history of diabetes and physical activity. Other parameters like BMI, glycosylated haemoglobin, fasting and post prandial blood sugar and fasting lipid profile were documented. Descriptive and bivariate analyses were carried out using the XLSTAT software (2020). Amongst 60 subjects, 65% were male and 93.3% were adults. 78% of subjects were following sedentary lifestyle and 40% had family history of diabetes. The results revealed that, obesity, family history of diabetes, uncontrolled glycemic status, sedentary lifestyles, and hypertension were prevalent among the Prameha subjects. The characterization of this risk profile and early detection of prameha by observing poorvarupa will contribute to designing more effective and specific strategies for screening and controlling Prameha in Maharashtra, India.

ABSTRACT

Older adults have difficulty monitoring their drug therapy in the first thirty days following hospital discharge. This transition care period may trigger hospital readmissions. The study aims to identify the factors associated with the readmission of older adults 30 days after discharge from the perspective of drug therapy. This is a cross-sectional study and hospital admission within 30 days was defined as readmission to any hospital 30 days after discharge. The complexity of the drug therapy was established by the Medication Regimen Complexity Index (MRCI).. Readmission risks were predicted by the "Readmission Risk Score - RRS". The multivariate logistic regression was used to identify factors associated with readmission within 30 days after discharge. Two hundred fifty-five older adults were included in the study, of which 32 (12.5%) had non-elective hospital readmission. A higher number of readmissions was observed with increased RRS value, suggesting a linear gradient effect. The variables included in the final logistic regression model were the diagnosis of cancer (OR=2.9, p=0.031), pneumonia (OR=2.3, p=0.055), and High MRCI (> 16.5) following discharge (OR=1.9, p=0.119). The cancer diagnosis is positively associated with hospital readmissions of older adults within 30 days

ABSTRACT

Abstract This study aimed to evaluate the incidence of medication errors over a period of one year in King Fahad Hospital Madina Saudi Arabia. This retrospective, cross-sectional study was conducted over a period of one year from 2018 January to 2018 December using patient's records. King Fahad Hospital in Madina was selected for the study center. Data collected include the number and types of errors, severity, location of errors, errors by profession, and errors occurred in the medical wards. Statistical analysis was carried out using statistical package for social science version22. A total of twenty- six hundred and fifty-eight medication errors were reported during the study period. Among the reports 2567(96.5%) of the errors were due to near misses, followed by transcribing errors 1597(60%), ordering errors 928(34.9%), duplicative therapy 765(28.7%), wrong dose 454(%). The most common procedures involve medication errors were wrong documentation 442(16.6%), duration 168(6.3%) wrong quantity 162(6.4%). Majority of the medication errors were contributed by physicians (99.2%) and 0.7% of them were due to the pharmacist. In conclusion study findings reported that yet some kind of medication errors has been under reported and it was common in most hospital, further studies with intervention programs needed to control the incidence of medication errors in a Saudi hospital

ABSTRACT

Abstract In the hospital environment, postoperative pain is a common occurrence that impairs patient recovery and rehabilitation and lengthens hospitalization time. Racemic bupivacaine hydrochloride (CBV) and Novabupi® (NBV) (S (-) 75% R (+) 25% bupivacaine hydrochloride) are two examples of local anesthetics used in pain management, the latter being an alternative with less deleterious effects. In the present study, biodegradable implants were developed using Poly(L-lactide-co-glycolide) through a hot molding technique, evaluating their physicochemical properties and their in vitro drug release. Different proportions of drugs and polymer were tested, and the proportion of 25%:75% was the most stable for molding the implants. Thermal and spectrometric analyses were performed, and they revealed no unwanted chemical interactions between drugs and polymer. They also confirmed that heating and freeze-drying used for manufacturing did not interfere with stability. The in vitro release results revealed drugs sustained release, reaching 64% for NBV-PLGA and 52% for CBV-PLGA up to 30 days. The drug release mechanism was confirmed by microscopy, which involved pores formation and polymeric erosion, visualized in the first 72 h of the in vitro release test. These findings suggest that the developed implants are interesting alternatives to control postoperative pain efficiently.

ABSTRACT

The reasons for the recently observed increase in the incidence of breast cancer in the Indian population are not clearly understood, but thought to be largely explained by westernization of lifestyles and changes in reproductive behavior, which characterize exposure to hormones. Our aim is to review the reproductive risk factors and comorbidities and evaluate the association between molecular subtypes of breast cancer. A hospital-based analytical case-control study was conducted among the breast cancer cases with controls in a multispecialty teaching hospital for a period of one year. Totally, 130 subjects were recruited and an interview was conducted using a structured questionnaire to obtain demographic and risk factor data, including tissue marker status (ER, PR and HER-2) obtained from case files. Data were analyzed with SPSS-20 version. Results: The highest age group reported in this study was 51- 60 years which has a 3.8 times increased risk compared to other age and the age group of 31- 40 have a decrease risk of 0.33. In this study, the percentage of post menopause (68%) and mothers not breastfeeding (10%) was higher in cases compared to controls and a noted increase in the risk of breast cancer with odds ratio (OR) of 2.745 (p= <0.0001) and 9.08 (p=0.01) respectively. Duration of breastfeeding showed significantly (p=<0.0001)) moderate positive correlation (r=0.549, 0.457, 0.418 and 0.636) for luminal A, luminal B, HER+, and triple negative respectively. This study found that all the reproductive risk factors do not have correlation with a molecular subtype of breast cancer except breastfeeding. Post menopause and breastfeeding were common factors associated with all people and could be modifiable to prevent the occurrence of breast cancer through lifestyle change

ABSTRACT

Abstract The objective of this study was to determine the prevalence and describe the factors associated with off-label drug use in an adult intensive care unit (ICU) of a Brazilian hospital. An analytical, cross-sectional, prospective study was conducted in the adult ICU population from March 2018 to May 2018. Off-label use of medication was classified by indication, dosage, route of administration, type and volume of diluent, and duration of administration. Most patients were female (57.89%), non-elderly (56.14%), and had a mean age of 54.44 ± 17.15 years. The prevalence of off-label drug use was 70.31%, but was not associated with the clinical severity of the patients. A statistically significant association was observed between label use of drugs and prescribing potentially inappropriate medicines (PIM). The most common reasons for off-label drug use were therapeutic indication (19.58%) and volume of diluent (23.30%). Drug administration by enteral tubes accounted for the largest number of off-label uses due to route of administration (90.85%). There was a higher prevalence of off-label use of systemic antimicrobials (14.44%) and norepinephrine (9.28%). Our study provided a broad characterization of off-label drug use in an adult ICU and showed why it is important for health professionals to evaluate the specific risks and benefits of this practice

ABSTRACT

Abstract The aim of this study is the association between the scores of disease activity, functional capacity and quality of life among patients diagnosed with rheumatoid arthritis, under clinical treatment at the Regional University Hospital of Campos Gerais - Wallace Thadeu de Mello and Silva. The sample was composed by volunteer patients, who freely underwent 3 research questionnaires. With the results of the survey, the disease activity score was correlated to the functional capacity and the quality-of-life scores. A mean of 3.87 and 1.2 was observed for the disease activity and the functional capacity scores, respectively, yet not achieving a correlation between those two variables. A strong correlation between the disease activity and the "functional capacity", "general health status" and "mental health" domains was found. The lowest average observed corresponded to "physical limitation", from the quality-of-life questionnaire. There was no statistically significant correlation between disease activity and functional capacity, although disease activity seems to affect the mental health, general health status and functional capacity of patients.

ABSTRACT

Drug-resistant Acinetobacter baumannii is a frightening reality. The aim of this study is to examine the expression profiles of blaOXA-51 gene in carbapenemases producing A. baumannii treated with imipenem/sulbactam combination. Carbapenemases producing A. baumannii was identified among clinical isolates of A. baumannii obtained from patients at Shahid Rajaee hospital, Gachsaran, Iran, from January to June 2018. Synergism testing of imipenem/sulbactam on carbapenemases producing A. baumannii was carried out by broth microdilution method. Eventually, the expression of blaOXA-51 gene was carried out to investigate the inhibitory properties of imipenem/sulbactam combination against carbapenemases producing A. baumannii using quantitative real time RT-PCR. Among A. baumannii isolates, 24% were carbapenemases producing A. baumannii. Imipenem/sulbactam combination revealed synergistic and partial synergistic effect for all tested isolates (FIC= 0.313-0.75). Finally, imipenem/sulbactam combination displayed significant down-regulation of blaOXA-51 gene in carbapenemases producing A. baumannii. Imipenem synergizes with sulbactam against carbapenemases producing A. baumannii by targeting of the blaOXA-51 gene.

Abstract: Objective: To establish the current situation of antimicrobial resistance and antibiotic consumption in Mexican hospitals. Materials and methods: Antimicrobial susceptibility data from blood and urine isolates were collected. Defined daily dose (DDD) of antibiotic consumption/100 occupied beds (OBD) was calculated. Results: Study period: 2016 and 2017. Of 4 382 blood isolates, E. coli and K. pneumoniae were most frequently reported, with antimicrobial resistance >30% for most drugs tested, only for carbapenems and amikacin resistance were <20%. A. baumannii had antimicrobial resistance >20% to all drugs. Resistance to oxacillin in S. aureus was 20%. From 12 151 urine isolates, 90% corresponded to E. coli; resistance to ciprofloxacin, cephalosporins and trimethoprim/sulfamethoxazole was >50%, with good susceptibility to nitrofurantoin, amikacin and carbapenems. Global median antimicrobial consumption was 57.2 DDD/100 OB. Conclusions: This report shows a high antimicrobial resistance level in Gram-negative bacilli and provides an insight into the seriousness of the problem of antibiotic consumption.


Resumen: Objetivo: Establecer la situación actual de la resistencia antimicrobiana y el consumo de antibióticos en hospitales mexicanos. Material y métodos:F Se colectaron datos de susceptibilidad antimicrobiana de aislamientos de sangre y orina. Se calculó la dosis diaria definida (DDD) del consumo de antibióticos/100 estancias. Resultados: Periodo de estudio de 2016 a 2017. De 4 382 aislamientos en sangre, E. coli y K. pneumoniae fueron las más frecuentes, con resistencia >30% a la mayoría de las drogas evaluadas; sólo para carbapenémicos y amikacina la resistencia fue <20%. A. baumannii tuvo resistencia >20% a todos los fármacos. La resistencia a oxacilina en S. aureus fue de 20%. De 12 151 aislamientos en urocultivos, 90% correspondió a E. coli; la resistencia a ciprofloxacina, cefalosporinas y trimetoprima/sulfametoxazol fue >50%, con buena susceptibilidad a nitrofurantoína, amikacina y carbapenémicos. La mediana del consumo global de antibióticos en DDD/100 estancias fue de 57.2. Conclusiones: Este reporte muestra el nivel elevado de resistencia en bacilos Gram-negativos y brinda una perspectiva de la gravedad del problema del consumo de antibióticos.

ABSTRACT

Resumen El trasplante hepático es la alternativa terapéutica indicada en pacientes con enfermedad hepática terminal para mejorar su sobrevida y calidad de vida. El objetivo de este estudio fue evaluar la calidad de vida relacionada con la salud (CVRS) de pacientes con cirrosis hepática antes y después de trasplante hepático. Se incluyeron 33 personas adultas que estaban en lista de espera para trasplante en la institución, se aplicó una serie de cuestionarios antes y después del trasplante: para evaluar la calidad de vida se utilizaron el LDQOL-1 (específico para enfermedad y trasplante hepático) y SF36 (para población general); para evaluar los síntomas depresivos y ansiosos, se utilizaron el BDI y STAI, respectivamente. Los resultados señalan una mejoría en la CVRS, así como disminución de los síntomas ansiosos y depresivos posterior al trasplante.


Abstract Liver transplantation is the therapeutic intervention of choice in patients with terminal liver disease to improve survival rate and quality of life. The aim of this study was to assess health related quality of life (HRQOL) in liver cirrhosis patients before and after liver transplantation. Thirty-three patients in waiting list for transplant in the institution were included; some questionnaires were applied before and after transplantation: to assess HRQOL were used LDQOL-1 (specific for liver disease and transplant) and SF36 (for general population); to assess depressive and anxious symptoms, BDI and STAI were used, respectively. Results showed an improvement in HRQOL and reduction in depressive and anxious symptoms after transplantation.

ABSTRACT

To estimate the frequency of the use of medicines listed in the Screening Tool to Alert Doctors to the Right Treatment (START) and Screening Tool of Older Person's Prescriptions (STOPP) criteria version 2 among the elderly. A cross-sectional study was conducted on elderly who were attended in medical clinic and cardiology sectors in a hospital in southern Brazil attended at a hospital from February through September 2016. A data-collection tool was used to obtain information on variables, such as demographic and clinical data, and medications used before and during the hospitalization period. The adequacy of the medicines taken was examined with regard to omission (START) or inappropriate use (STOPP). This study was approved by the Research Ethics Committee of the University of Southern Santa Catarina. A total of 307 subjects were included in the final sample. The mean age was 75.2 years (SD = 8; range 65-102). Of the total, 93.5% had had at least one potential prescribing omission (PPO) listed in the START criteria, whereas 95.4% used at least one medicine of the STOPP criteria. PPO was significantly associated with lower mean age (74.9 years, SD = 7.9 versus 79.0 years, SD = 8.8) among the elderly who did not have PPOs detected by the START criteria (p-value=0.03). Furthermore, PPO was associated with longer hospital stay (18 versus 9 days; p-value=0.03). This study revealed inadequate prescription affecting 99.3% of the participating patients. To the best of our knowledge, this was the first to use the START and STOPP criteria, version 2, in Brazil.

Este ordenamiento facilitará hacer una gestión basada en resultados, proceso que evitará la duplicidad de esfuerzos.

Resumo: No Brasil, a convivência público-privado no financiamento e na prestação do cuidado ganha nítidos contornos na assistência hospitalar. Os arranjos de financiamento adotados pelos hospitais (Sistema Único de Saúde - SUS e/ou planos de saúde e/ou pagamento particular) podem afetar a qualidade do cuidado. Alguns estudos buscam associar a razão de mortalidade hospitalar padronizada (RMHP) a melhorias na qualidade. O objetivo foi analisar a RMHP segundo fonte de pagamento da internação e arranjo de financiamento do hospital. Analisaram-se dados secundários e causas responsáveis por 80% dos óbitos hospitalares. A RMHP foi calculada para cada hospital e fonte de pagamento. Hospitais com desempenho pior que o esperado (RMHP > 1) foram majoritariamente públicos de maior porte. A RMHP nas internações SUS foi superior, inclusive entre internações no mesmo hospital. Apesar dos limites, os achados indicam iniquidades no resultado do cuidado. Esforços voltados para a melhoria da qualidade de serviços hospitalares, independentemente das fontes de pagamento, são prementes.


Abstract: In Brazil, the combined presence of public and private interests in financing and provision of healthcare services stands out clearly in hospital care. Financing arrangements adopted by hospitals (the public Brazilian Unified National Health System - SUS and/or health plans and/or out-of-pocket payment) can affect quality of care. Studies have analyzed the hospital standardized mortality ratio (HSMR) in relation to quality improvements. The objective was to analyze HSMR according to source of payment for the hospitalization and the hospital's financing arrangement. The study analyzed secondary data and causes that accounted for 80% of hospital deaths. HSMR was calculated for each hospital and payment source. Hospitals with worse-than-expected performance (HSMR > 1) were mostly large public hospitals. HSMR was higher in the SUS, including between admissions in the hospital. Despite the study's limitations, the findings point to inequalities in results of care. Efforts are needed to improve the quality of hospital services, regardless of the payment sources.


Resumen: En Brasil, la convivencia público-privada en la financiación y en la prestación del cuidado empieza a definirse nítidamente en la asistencia hospitalaria. Los acuerdos de financiación adoptados por los hospitales (Sistema Único de Salud - SUS y/o planes de salud y/o pago particular) pueden afectar a la calidad del cuidado. Algunos estudios buscan asociar la razón de mortalidad hospitalaria padronizada (RMHP) a mejorías en la calidad. El objetivo fue analizar la RMHP según la fuente de pago del internamiento y acuerdos de financiación del hospital. Se analizaron datos secundarios y causas responsables de un 80% de los óbitos hospitalarios. La RMHP se calculó para cada hospital y fuente de pago. Los hospitales con un desempeño peor que el esperado (RMHP > 1) fueron mayoritariamente públicos y con un mayor número de pacientes. La RMHP en los internamientos SUS fue superior, incluyendo internamientos en el mismo hospital. A pesar de los límites, los hallazgos indican inequidades en el resultado del cuidado. Son necesarios esfuerzos dirigidos a la mejoría de la calidad de servicios hospitalarios, independientemente de las fuentes de pago de los mismos.

ABSTRACT

Efficiency of the hospitals affects the price of health services. Health care payments have equity implications. Evidence on hospital performance can support to design the policy; however, the recent literature on hospital efficiency produced conflicting results. Consequently, policy decisions are uncertain. Even the most of evidence were produced by using data from high income countries. Conflicting results were produced particularly due to differences in methods of measuring performance. Recently a management approach has been developed to measure the hospital performance. This approach to measure the hospital performance is very useful from policy perspective to improve health system from cost-effective way in low and middle income countries. Measuring hospital performance through management approach has some basic characteristics such as scoring management practices through double blind survey, measuring hospital outputs using various indicators, estimating the relationship between management practices and outputs of the hospitals. This approach has been successfully applied to developed countries; however, some revisions are required without violating the fundamental principle of this approach to replicate in low- and middle-income countries. The process has been clearly defined and applied to Nepal. As the results of this, the approach produced expected results. The paper contributes to improve the approach to measure hospital performance.

ABSTRACT

FUNDAMENTO: Os gastos com cirurgia de revascularização do miocárdio (RVM) e angioplastia coronariana (AC), representaram importante ônus para o SUS. OBJETIVO: Analisar gastos do SUS com RVM e AC e sua performance nos hospitais do Estado do Rio de Janeiro (ERJ), de 1999 a 2008. MÉTODOS: As informações provieram das AIH pagas dos hospitais com mais de 100 revascularizações. As taxas de letalidade foram ajustadas por modelos Poisson (covariáveis idade, dias de permanência no hospital e gasto em UTI). Foram construídos índices de gasto médio relativo, dividindo-se o valor médio da fração de gasto em cada hospital pelo gasto médio no ERJ, em dólares. Para análise estatística empregou-se o Stata. RESULTADOS: Foram pagas 10.983 RVM e 19.661 AC em 20 hospitais nos 10 anos, com valores médios de US$ 3.088,12 e 2.183,93, respectivamente. A taxa de letalidade nas RVM flutuou de 9,2 por cento-1999 para 7,7 por cento-2008, com valores extremos de 5,0 por cento-9,2 por cento e nas AC de 1,6 por cento-1999 para 1,5 por cento-2008, com valores extremos de 0,9 por cento-2,3 por cento. Os hospitais diminuíram a realização de RVM e duplicaram a de AC. Idade, tempo de internação e gastos em UTI correlacionaram-se significativamente com a letalidade nas RVM e AC pagas no ERJ. Em média, os gastos com os serviços hospitalares representaram 41 por cento do total das RVM e 18 por cento das AC, e os com as órteses e próteses, 55 por cento das AC e 28 por cento nas RVM. CONCLUSÃO: Evidencia-se necessidade de melhorar a qualidade do atendimento das instituições que realizam RVM e AC pagas pelo SUS.


BACKGROUND: Expenses with coronary artery bypass grafting (CABG) surgery and coronary angioplasty (CA) represented a significant cost to SUS. OBJECTIVE: To analyze SUS expenses with CABG and CA and their performance in hospitals in the state of Rio de Janeiro (SRJ), from 1999 to 2008. METHODS: The information came from paid HAA in hospitals with more than 100 revascularization procedures. Mortality rates were adjusted by Poisson (with covariates age, length of hospital stay and ICU expenses). Mean relative cost indices were calculated by dividing the mean value of the cost fraction spent in each hospital by the mean expense in the SRJ in U.S. dollars. Stata software was used for statistical analysis. RESULTS: A total of 10,983 CABG and 19,661 CA were paid by SUS in 20 hospitals during the 10 years, with mean values of US$ 3,088.12 and 2,183.93, respectively. The mortality rate in CABG varied from 9.2 percent-1999 to 7.7 percent-2008, with ranges of 5.0 percent-9.2 percent and in CA, from 1.6 percent-1999 to 1.5 percent-2008, with ranges of 0.9 percent-2.3 percent. The hospitals decreased the number of CABG procedures and doubled CA procedures. Age, time of hospital stay and ICU costs significantly correlated with lethality in CABG and CA paid by SUS in the SRJ. On average, hospital service costs represented 41 percent of the total cost of CABG and 18 percent of CA, and with ortheses and prostheses, 55 percent in CA and 28 percent in CABG. CONCLUSION: It is clear the need to improve the quality of healthcare service in institutions that perform CABG and CA paid by SUS.

ABSTRACT

FUNDAMENTO: Aferição da prática clínica brasileira em pacientes com síndrome coronariana aguda, em hospitais públicos e privados, permitirá identificar os hiatos na incorporação de intervenções clínicas com benefício comprovado. OBJETIVO: Elaborar um registro de pacientes portadores do diagnóstico de síndrome coronariana aguda para aferir dados demográficos, morbidade, mortalidade e prática padrão no atendimento desta afecção. Ademais, avaliar a prescrição de intervenções baseadas em evidências, como a aspirina, estatinas, betabloqueadores e reperfusão, dentre outras. MÉTODOS: Estudo observacional do tipo registro, prospectivo, visando documentar a prática clínica hospitalar da síndrome coronária aguda, efetivada em hospitais públicos e privados brasileiros. Adicionalmente, serão realizados seguimento longitudinal até a alta hospitalar e aferição da mortalidade e ocorrência de eventos graves aos 30 dias, 6 e 12 meses. RESULTADOS: Os resultados serão apresentados um ano após o início da coleta (setembro de 2011) e consolidados após a reunião da população e dos objetivos posteriormente almejados. CONCLUSÃO: A análise desse registro multicêntrico permitirá projetar uma perspectiva horizontal do tratamento dos pacientes acometidos da síndrome coronariana aguda no Brasil.


BACKGROUND: Assessing the Brazilian clinical practice in patients with acute coronary syndrome, in public and private hospitals to identify gaps in the incorporation of clinical interventions with proven benefit. OBJECTIVE:To develop a registry of patients diagnosed with acute coronary syndrome to assess demographics, morbidity, mortality, and standard practice in the care of this condition. Besides, to assess the prescription of evidence-based interventions such as aspirin, statins, beta blockers and reperfusion, among others. METHODS: Registry-type prospective observational study intended to document hospital clinical practices of acute coronary syndrome in public and private hospitals in Brazil. In addition, longitudinal follow-up will be held until discharge and measurement of mortality and occurrence of serious events at 30 days, 6 and 12 months. RESULTS: The findings will be presented one year after the start of collection (September 2011), and consolidated after a meeting with the population to discuss the objectives sought. CONCLUSION: The analysis of this multicenter registry will design a horizontal perspective for the treatment of patients suffering from cardiovascular disease in Brazil.

OBJETIVO: Elaborar un registro de pacientes portadores del diagnóstico del síndrome coronario agudo para comprobar los datos demográficos, morbilidad, mortalidad y la práctica estándar en la atención de esa enfermedad. Además, evaluar la prescripción de intervenciones basadas en evidencias, como la aspirina, estatinas, betabloqueantes y reperfusión, entre otras. MÉTODOS: Estudio observacional de tipo registro prospectivo, con el fin de documentar la práctica clínica hospitalaria del síndrome coronario agudo, que se ha llevado a cabo en hospitales públicos y privados brasileños. Por añadidura, serán realizados el seguimiento longitudinal hasta el alta, la comprobación de la mortalidad y la aparición de eventos graves a los 30 días, 6 y 12 meses. RESULTADOS: Los resultados se presentarán un año después del inicio de la recopilación (setiembre de 2011), y consolidados después de la reunión de la población y de los objetivos posteriormente deseados. CONCLUSIÓN: El análisis de ese registro multicéntrico permitirá proyectar una perspectiva horizontal del tratamiento de los pacientes afectados con el síndrome coronario agudo en Brasil.
